The best text I've seen on SPICE is The SPICE Book by Andrei Vladimirescu, published in 1994 through John Wiley & Sons, Inc., ISBN 0-471-60926-9.
Notable for including many models, such as OP-AMP macro models, is Computerized Circuit Analysis With SPICE by Thomas W. Thorpe, published in 1991 also through John Wiley & Sons, Inc., ISBN 0-471-55164-3.
For general IC design information I highly recommend Analysis and Design of Analog Integrated Circuits, 3d ed. by Gray and Meyer, published in 1992 by John Wiley & Sons, Inc., ISBN 0-471-57495-2.
